The service provides a dedicated local point of contact in Roscommon to support people with dementia and their families to live well at home.
The Community Dementia Support Nurse, Fiona Beattie, will provide:

information and support to empower people to live well with dementia

individual support at key transition times, including visiting people at home when necessary.

tailored assessments and care plans and regular care reviews

Information about accessing services and supports and navigating the health and social care system

Fiona is available on Monday, Tuesday and Wednesday each week.
GP’s and other health & social care professionals can refer into the service by using the referral form.
This service is part of the There's No Place Like Home project in partnership with the HSE, Genio and The Alzheimer Society of Ireland.
