Dementia Adviser

Our Dementia Advisers work with people with dementia, their families and carers to provide a highly responsive and individualised information and signposting service. This service is free and confidential. Dementia Advisers are currently doing home visits in line with public health advice and COVID 19 restrictions.  Dementia Advisers continue to provide support over the phone

How to access this service?

Contact our home care coordinator above. Our home care coordinators will carry out an initial assessment on clients.

To help with this assessment we ask that a referral form is completed and sent to the home care service you are interested in.

Our day care centres provide dementia specific, person centred care to meet the needs of the person with dementia in a warm, welcoming and safe environment while providing support and stimulation

How to access this service?

People with dementia, their families and healthcare professionals can contact the day centre they are interested in.

We ask that a referral form is completed and this form requires confirmation of a diagnosis of dementia from a GP or consultant.

Contact the day care centre you are interested in to talk about the service and to send your completed referral form.

Online Family Carer Training

We offer training to family carers looking after a loved one with dementia. Family carers can either be the primary carers for the person with dementia or those indirectly involved in caring. Please note there is a waiting list for this service.

We run support groups for families affected by dementia around Ireland.

Online Carer Support Groups

We run support groups for families affected by dementia around Ireland. The support groups provide an opportunity to meet others in a similar situation, to share stories and experiences, and access information and practical advice in a relaxed, understanding and supportive environment.

Day Care at Home

The Day Care at Home service provides an alternative model of care and support to our clients and families. ASI staff visit the persons home and engage in personalised activities with them in their home.

Social Activities

Note:

Age Action’s Care and Repair does small DIY jobs, free of charge, for older people to help them maintain their homes and their independence.

Note:

The Rainbow Café is specifically for older LGBT+ persons, proactive allies, and healthcare professionals supporting older persons.
The space is dementia-inclusive and those living with dementia, along with family carers and supporters, are encouraged to attend and will be assisted to do so in any way possible.

Note:

Social Prescribing is a HSE run service that connects people with supports and activities in the community to strengthen their health, wellbeing and quality of life. Social prescribing is about supporting health and wellbeing by linking people into local, community-based activities such as exercise, art, reading and gardening

Siel Bleu Physical Activities

Siel Bleu holds a range of physical activity sessions which are open for people with dementia and their carers to attend. The cost per session is €5. Siel Bleu also offers one to one classes which can be held in people’s homes at a cost of €30 per session. The Memory Clinic is a specialised service for people with memory loss, changes in cognitive function and dementia.
